Veterans looking for monument names

Jeff Davis County Veterans Association (JDCVA) is informing anyone that would like to have a veteran’s name inscribed on the memorial wall at Veterans Square Park. The cost if $40 per engraving and payable by check or money order, only to Jeff Davis County Veterans Association, Inc. The JDVCA does not accept cash or credit cards at this time. A separate form is required for each name inscribed on the memorial.

Fill out each part of the form as completely and accurately as possible. The completed form and accompanying check or money order may be returned via one of three methods: by mail to JDCVA, Post Office Box 263, Hazlehurst, GA 31539; in person at Hazlehurst-Jeff Davis Chamber of Commerce, Big House; in person at Hazlehurst City Hall on South Cromartie St.
As JDCVA is a 501(c)(3) corporation, the funds are tax deductible.
